There is no denying that healthcare is inherently risky. Both familiar and unexpected risks continue to exist, and the clinical risk universe is broad and complex. Gaps in care delivery can affect patient safety, care quality, and organizational performance, and it can be overwhelming to think about filling all gaps at once.
