DESCRIPTION:CMS is expanding its use of mandatory\, episode-based payment models through the upcoming Comprehensive Care for Joint Replacement Expanded (CJR-X) Model\, scheduled to begin in October 2027. This model shifts greater financial accountability to hospitals\, with performance tied to cost and quality outcomes across defined episodes of care. CJR-X is projected to generate approximately $725 million in savings for CMS over its first five years and doesn’t currently have a defined end date. CJR-X is expected to be finalized in the Inpatient Prospective Payment System (IPPS) final rule in late July or early August 2026. \n\n\n\nPreparing for participation in CJR-X will require an understanding of the model’s structure\, performance expectations\, and potential implications for hospitals.
