CJR-X: What to Know & How to Prepare

CMS is expanding its use of mandatory, episode-based payment models through the upcoming Comprehensive Care for Joint Replacement Expanded (CJR-X) Model, scheduled to begin in October 2027. This model shifts greater financial accountability to hospitals, with performance tied to cost and quality outcomes across defined episodes of care. CJR-X is projected to generate approximately $725 million in savings for CMS over its first five years and doesn’t currently have a defined end date. CJR-X is expected to be finalized in the Inpatient Prospective Payment System (IPPS) final rule in late July or early August 2026.
Preparing for participation in CJR-X will require an understanding of the model’s structure, performance expectations, and potential implications for hospitals. Organizations that begin evaluating their readiness early may be better positioned to navigate the transition. This session will provide an overview of CJR-X, outlining participation requirements and key considerations for preparing to perform successfully under the model.
