Navigating TEAM: Strategic Positioning for SNFs

On January 1, 2026, CMS launched the Transforming Episode Accountability Model (TEAM), a mandatory bundled payment model for over 700 acute care hospitals across the country. TEAM places new financial accountability on hospitals for the cost and quality of care delivered across specific 30‑day episodes of care. As healthcare organizations adjust to these expectations, senior living and long-term care providers play an increasingly important role in supporting care coordination, outcomes, and efficiency over the episode of care. This webinar will explore how senior living and long-term care providers are supporting effective transitions of care by delivering the right level of care in the most appropriate setting. This session will also discuss the overall view of value-based care in the post-acute care environment.
